ABSTRACT The present article, brings about a critical discussion on public policies in gender violence, and in so doing, intends to introduce the reader, into the basic notions of the constructivist theory and its articulation with the psychoanalytic ideas as postulated by Freud. The inter-relation of both theories of knowledge has to do with their fundamental common denominator: the deconstruction of fixed models and paradigms. In this sense, this paper, despite having its main focus on violent male practices, has as bigger aspiration, to highlight a way of thinking and practicing psychoanalytical clinical work as work that goes beyond the private individual clinical practice. The author also proposes to differentiate between modes of violent practices, coining the expression identitary distress. The paper, although originated from the study of violent male practices, can be used in a more ample form, in the sense of contributing to the understanding of phenomena pertaining to a wider range of experiences of social, individual and institutional spheres. Trabalho com a hipótese de que os efeitos destas políticas tendem a encandilar e perpetuar aquilo mesmo que pretendem combater. De que violência falamos? O desprazer resultante da intensidade pulsional a que o aparelho psíquico fica submetido, é vivido como desamparo, pondo em risco a integridade deste, assim quanto a do sujeito.

This qualitative study focused on women who had experienced both types of delivery, in three maternity hospitals in Rio de Janeiro, Brazil one public, one fully private, and another private under an outsourcing agreement with the public health system. The study showed that variations in public and private service models result in different types of delivery care and different relations with staff, and are reflected in different birthing experiences for the women. However, a critical gender perspective shows that in both cases, the service models reproduce the medicalization of childbirth and women's submission as objects in the birthing process. Although this is manifested in different ways in the three groups, the end result is to reduce the range of care and the possibility of women's empowerment during childbirth.
